February is American Heart Month. We understand that cardiac patients and their caregivers deserve a specialized level of care. That is why we developed an evidence-based clinical approach to effectively manage chronic cardiac illness.
Through our commitment to excellence, we are proud to deliver cardiac care best practices in the comfort and convenience of a patient’s home, wherever that may be.
Services for Patients with Cardiac Complications of CABG/Congestive Heart Failure/Hypertension/PVD/ ASHA/MI/Pacemaker may include but are not limited to:
- Cardio-respiratory assessment
- Assessment of signs and symptoms of CHF including edema and dyspnea/use of incentive spirometer
- Management of medications
- Teaching and monitoring of gradual exercise programs including target heart rate
- Teaching use of O2 safety and PRN O2 saturation
- Teaching on need for daily weights; recording and reporting increase to MD
Triggers for Eligibility may include:
- Frequent ER visits/hospitalizations
- Escalation of symptoms (chest pain, shortness of breath, dizziness)
- Frequent medication changes
- New York Heart Association (NYHA) Class 1-4
- Oxygen use • Declining functional status
- Anxiety/depression/suffering
In addition, we work with community partners to provide services that meet each patient’s needs. This continuum of care may include:
- Skilled nursing and rehabilitation services
- Assisted living care
- Outpatient rehabilitation
- Hospice care
