Heartland Home Health Care and Hospice is joining a nationwide effort by other national, state and community organizations to highlight the importance of advance healthcare decision making. National Healthcare Decisions Day (NHDD) is April 16, and Heartland will be providing information and tools for the public to use when talking about their healthcare wishes with their family, friends and healthcare providers.
For 2017, NHDD is being celebrated the week of April 16-22. Our hope is that more people in the community will plan ahead for their healthcare. It is difficult to have these important discussions with your family and friends, but if we help them to plan, we are hoping that fewer families and healthcare providers will have to make the difficult decisions in the absence of guidance from the patient. The ultimate goal is to honor the patient’s wishes and the first step is to understand what the patient’s wishes are.
Heartland’s mission is to support the patient and family wherever they are in their health care journey and to respect their goals and values. Heartland Hospice provides specialized care that meets the physical, emotional and spiritual needs of patient and loved ones facing the challenges of an advanced illness. At Heartland Home Health, we proudly provide health care at home, whether home is the house you’ve lived in for years, or a senior retirement setting such as an independent or assisted facility.
